Session Title: Where data meets compassion: improving the patient experience
Patients’ unmet social needs greatly impact hospital economics including patient satisfaction, staff productivity and clinical outcomes. What happens when patients can't get a ride to the hospital for treatments every day? What if they need low sodium meals delivered at home? What if they’re traveling from out of town and need affordable lodging? We know that health care delivery and outcomes are improved when a patients has the support services they need.  Today, however,  providers don’t have the talent, training or tools to identify and address the full breadth of these social needs.
